Abstract

Progress reports for the year 2017 compiled by executive healthcare authorities of 11 constituent territories of the Russian Federation that are part of the Northwestern Federal District were analysed. The child population aged 0−17 years amounted to 2,537,133 people; the number of paediatric oncological beds was 174 (0.7 per 10,000 children aged 0−17 years); the average duration of bed occupancy was 290.7 days a year. Four (36.4 %) constituent territories lacked paediatric oncology departments and children with cancer were allocated oncological beds in other paediatric departments, while still four territories (36.4 %) lacked even these beds. The number of doctors providing medical care to children with cancer was 38, of them only 27 (71 %; 0.1 per 10,000 children aged 0−17 years) were certified paediatric oncologists. Two (18.2 %) constituent territories had no paediatric oncologists whatsoever. The incidence of malignant tumours was 15.6, the prevalence was 92.6, while the mortality rate was 2.6 per 100,000 children aged 0−17 years; one-year mortality was 4.5 %. The number of cancer cases identified during preventive examination was 11 (2.8 %). 160 (40.3 %) primary care patients were referred to federal medical institutions, while 6 (1.5 %) primary care patients went for treatment abroad. Incidence and mortality stayed at a rather low level, which, however, indicates diagnostic errors and lack of reliable catamnestic data. The share of patients referred for treatment to federal medical institutions is not very large; however, to interpret this indicator in terms of patient routing, one would have to analyse the medical history of all the patients. It can be rightfully argued that medical care for children with cancer in the Northwestern Federal District is at an acceptable level but requires improvement.
